All students participating in sports are required to fill out and submit the EVMS Athletics Participation Forms Packet to their coach or the school office by the end of the first week of any sport's season. Once this packet is complete and on file in the school office it is good for subsequent sports seasons in that same school year.

There is a $35 fee per sport ($50 for Football).  These fees will be accessed in PowerSchool, payable by Credit Card. If you are unable to pay the fee or need to pay at a later date please contact the school office (970) 328-8910. Please do not let this fee discourage you or your child from participating in EVMS Athletics. 

Please Note: 

• The Physical should be signed and dated by a physician and is good for 365 days. If the year lapses before the end of the season a new physical form must be completed to continue participation. Please call the Front Office to verify if you think that your athletes physical hasn't expired. The physician may use their own form if they choose, but please ensure that the student's name and the date of the exam is clearly listed.

• Please know that BOTH the athlete and the parent/guardian must sign BOTH the No Contact Agreement and the Sports Contract.
