All students participating in sports are required to fill out and submit the EVMS Athletics Participation Forms Packet to their coach or the school office by the end of the first week of any sport's season. Once this packet is complete and on file in the school office it is good for subsequent sports seasons in that same school year.

There is a $35 fee per sport per year.  These fees may be paid by Check (made out to EVMS with the athlete's name and sport/camp listed in the memo area), Cash, or Credit Card via MySchoolBucks.com. If you are unable to pay the fee or need to pay at a later date please contact the school office (970) 328-8910. Please do not let this fee or difficulty attaining a physical discourage you or your student from participating in EVMS Athletics. We have many resources available to allow your student to participate.

Please Note: 

• The Physical should be filled out by a physician and is good for 365 days. If the year lapses before the end of the season a new physical form must be completed to continue participation. Please call the Front Office to verify if you think that your athletes physical hasn't expired. The physician may use their own form if they choose, but please ensure that the student's name and the date of the exam is clearly listed.

• Please know that BOTH the athlete and the parent/guardian must sign BOTH the No Contact Agreement and the Sports Contract.
